The General Assembly of the State of Tennessee requires all entering students to complete an immunization compliance process before attending UT Martin. Immunization information must be completed, uploaded, submitted, and approved seven business days before the start of each semester.
All immunization documentation must be submitted through Med+Proctor, the UTM immunization portal. Med+Proctor is a free service for students, but if you wish to have lifetime access to your records, you can pay $10. This is your decision.

Measles, Mumps, Rubella (MMR) Requirements
Must provide proof of two immunizations of Measles, Mumps, and Rubella (MMR) vaccines or laboratory evidence of immunity to all three diseases.
Exemptions include students born before 1/1/1957, on-line only students, medical contraindication verified by a healthcare provider, or religious exemption. Students should submit the Medical and/or Religious Exemption Form to Med+Proctor.
Varicella (Chickenpox) Requirements
Must provide proof of two doses of varicella (chickenpox), or documented proof of having the disease by healthcare provider, or laboratory evidence of immunity to the disease.
Exemptions include students born before 1/1/1980, on-line only students, medical contraindication verified by a healthcare provider, or religious exemption. Students should submit the Medical and/or Religious Exemption Form to Med+Proctor.
Hepatitis B Requirements
Recommended but not required. The General Assembly of the State of Tennessee mandates that each public or private postsecondary institution in the state provide information concerning hepatitis B infection to all students entering the institution for the first time. Tennessee law requires that such students complete and sign a wavier form provided by the institution. Waiver is located on the Med+Proctor portal.
Meningococcal Disease (Meningitis) Requirements
Meningitis (Menactra®, Menveo®, and MenHibrix®): Required for any students who are younger than 22 years of age and who will be living in on-campus housing MUST provide proof of receiving a dose of quadrivalent conjugate vaccine at greater than or equal to 16 years of age. If your initial vaccine for meningitis was given prior to the age of 16, immunization requirements will not be met unless and additional booster vaccine is given after your 16th birthday. Failure to provide adequate documentation will prevent you from moving into on-campus housing upon your arrival.
Meningococcal Serogroup B Vaccine is recommended but not required and will not be accepted in place of the Meningococcal conjugate vaccine requirement.
A student who has been exempted from a particular vaccination must comply with the immunization requirements for any vaccines from which he or she has not been exempted.
