The General Assembly of the State of Tennessee requires all entering students to complete an immunization compliance process before attending UT Martin. Immunization information must be completed, uploaded, submitted, and approved seven business days before the start of each semester.

The immunization compliance process can be satisfied in a variety of ways including proof of vaccination, positive (+) titers, medical or religious waivers, other documentation by a medical professional, a valid Military ID or DD214.

Follow these steps to meet immunization requirements at UTM:

Step One: Review Required and Recommended Immunizations

All students are required to submit proof of:

  • Measles, Mumps, and Rubella (MMR) Vaccine – Proof of immunizations with two doses at least 28 days apart or positive (+) antibody titers for any student born after January 1, 1957 is required.
  • Varicella (Chickenpox) Vaccine – Proof of immunizations with two doses at least 28 days apart, a positive (+) antibody titer, or documented history of disease for all students born after January 1, 1980 is required.

Students living on campus are ALSO required to submit proof of:

  • Meningitis ACWY Vaccines – All new incoming students who are under 22 years of age and will be residing in on-campus housing, must have documentation of the Meningococcal Serogroup A, C, W, and Y Vaccine given on or after their 16th birthday or they will not be permitted to move into on-campus housing.

Other currently recommended, but not required, immunizations include:

  • Hepatitis B Vaccine:
  • Human Papillomavirus (HPV) Vaccine
  • Meningococcal Serogroup B Vaccine
  • Primary Polio series
  • Tetanus, Diphtheria, and Pertussis Vaccine (Tdap)
  • Covid-19 vaccine
  • Influenza vaccine

Students can request a religious or medical exemption to the immunizations requirement by completing and submitting the immunizations exemption form. Medical exemptions require the signature of a health care provider.

Step Two: Review Other Important Immunization Information

What is required?

  • A copy of medical records documenting each required vaccine listed on the immunization form or proof of immunity to disease must be provided.

Alternative Documentation

  • As an alternative to an official medical record, this immunization form can be completed with the healthcare provider’s signature and office stamp in the appropriate place and uploaded into the Student Wellness Portal.
  • Active service members or veterans attending UTM can submit their Military ID or DD214 to satisfy immunizations requirements.

Exemptions

  • If you are exempt from certain immunizations for medical reasons, you must upload a signed and stamped statement from a healthcare provider to the Student Wellness Portal.
  • If you have a religious exemption, you must complete, sign, and upload this form to the Student Wellness Portal.
  • If you are enrolled in online-only courses with no experiential component, please contact your academic advisor IF your account shows non-compliant or you have an immunization hold.
  • If you believe you qualify for another exemption, please email shcs@utm.edu with the details regarding your exemption.
  • Note that students enrolled in any allied health professions program may not have the same exemptions to immunization requirements under certain circumstances. 
Step Three: Submit Your Immunizations

Submit or update immunization information via the Student Wellness Portal

Remember to:

  • Have appropriate documentation completed and available for upload as noted in Step 2.
  • Upload appropriate documentation to the Student Wellness Portal.
  • Input correct dates of required and recommended vaccines under the immunization tab on the Student Wellness Portal to prevent a delay in approving your immunizations and removing any immunization holds on your account.

Frequently Asked Questions

I’m moving onto campus and I’m not compliant. What do I need to do?
  • Have appropriate documentation completed and available for upload as noted in Step 2, specifically the meningococcal vaccination information.
  • Upload appropriate documentation to the Student Wellness Portal.
  • Input correct dates of required and recommended vaccines under the immunization tab on the Student Wellness Portal to prevent a delay in approving your immunizations and removing any immunization holds on your account.’
  • During peak season, it may take up to three days for your immunizations to process. Please allow sufficient time prior to move-in to get your immunizations uploaded and processed.
  • If you believe you have everything uploaded correctly and you are still listed as non-compliant, please contact the Student Health and Counseling Services Office at 731-881-7750 or SHCS@utm.edu.

Where can I obtain a copy of my immunization records?

The following locations may have copies of your immunization records that you can upload to the Student Wellness Portal.

  • High school
  • Local health department
  • Primary care physician
  • Previous college or university
  • Parent or guardian

Please Note: For currently enrolled students, UTM Student Health and Counseling Services does not maintain a copy of immunization records to be released to the students. It is important for all students to maintain their own copy for their records.

I’m unable to submit my immunizations online. What do I do?

If you are unable to access your Student Wellness Portal, please contact the IT help desk at 731-881-7900.

If you are having an issue with accessing the immunizations page within your Student Wellness Portal, contact Student Health and Counseling Services at 731-881-7750.

I’m exempt from immunizations. How do I still satisfy the Immunization Requirements?

What if I’m only taking classes Online or I’m a Dual Enrollment student?

Students enrolled in a course of study that is exclusively online and do not attend any in-person classes on any UTM campus are exempt from submitting immunization information. If at any time students who were previously online-only register to attend any in-person classes, they will then be subject to the in-person immunization requirements as per above.

Dual Enrollment students that attend class exclusively through their local education institution are also exempt from providing immunization information. However, Dual Enrollment Students that attend classes on any of the UTM campuses are subject to the in-person immunization requirements listed above.

If you believe you should be exempt from vaccinations due to taking online-only classes, please contact your academic advisor.

I have an immunization hold on my account. How do I get it removed?

Students who do not satisfy their immunization requirements prior to the start of each semester will get a registration hold placed on their account. This will prohibit such things as registering for classes and adding/dropping classes.

To remove the hold, students must satisfy the immunization requirements as provided above. Remember, it may take 24-72 hours for the immunizations to be processed once submitted.

What happens if I don’t submit my immunization information on time?

Residential students—students living on campus—will not be allowed to move in until they show proof of meningococcal vaccination as listed above. All students that do not provide documentation that satisfies the requirements for MMR and Varicella will have an immunization hold placed on their student account after the first 14 days of classes. This hold will prevent students from registering and adding/dropping classes.
